CREDIT CARD  PAYMENT AUTHORIZATION FOR DATE CHANGE 

Before making the payment using your credit card for date change or re-issue, you have to authorize us to charge your credit card for the purchase by completing the following authorization text in your email.

 1) Email Text as under : (Copy this text to email body and fill up all required info).
 2) Send a scanned copy of your ID proof (Driver Licence, passport, other valid ID documents)
 3) Send a scanned copy of front of Credit Card.
 4) Call us and give us your credit card number over phone(DO NOT TYPE/SEND FULL CC NO IN EMAIL)
 5) After we verify the details, we will put date change or re-issue request to airlines.
  6) We may need more info if asked by the ticket issuing authority.

                      I understand that a Travel Network or a major airline carrier or travel partners name will appear on my credit card statement with different charge amount, total amount equal to the above authorized amount. If my credit card is charged a slightly larger amount than I have authorized due to ticket fare type, Konark Travel will send a refund to me by check for the overcharge amount within 2 to 4 weeks of ticket issue date.I'm aware that in regards to this authorization, if I dispute the above amount later on OR if the bank/credit card issuer reject the payment, I will be liable to the maximum collection fees allowed by the laws of the state of residence in addition to the amount authorized by me to be charged by a collection agency authorized by Konark Travel.If my credit card is charged and ticket is not issued because of any reasons (fare expired, airlines cancelled booking,duplicate booking, etc), transaction will be cancelled/voided within 24-48 hours. I am aware that fare paid is not guaranteed unless ticket is issued by airlines.
